On Monday, 8/1/2022, a new eForm will be launched that will replace the GWS-11 form Change in Owner Name/Mailing Address used to update owner name and/or contact information for individual well permits.
Form GWS-11 (Change in Owner Name/Mailing Address) will no longer be available on our website, but DWR will continue to accept this form for a 60-day transition period of August through September. GWS-11 forms will no longer be accepted after September 30, 2022.
Title Company representatives, that will file name change information for well permits in connection with a real estate transaction, will need to create a registration with DWR to be able to submit name change information. Title Companies representatives can register beginning July 18, 2022, using this link: Title Company Registration. Please allow 1-2 business days for processing registration requests.
