We have released new eForms to update owner name and/or contact information. Owners of valid well permits are required pursuant to Section 37-90-143 of the Colorado Revised Statutes to file an update with the Colorado Division of Water Resources (DWR) regarding any change of owner name and/or contact information. There is no filing fee to submit this form. Please note, you will need to have an email address and create an account to file the appropriate eForm.
- The “Change of Owner Name/Contact Information eForm” should be used by the new well owner or the well owner’s attorney/authorized agent, to update owner name and/or contact information for individual well permits.
- The “Title Companies - Change of Owner Name/Contact Information eForms” are to be used by title companies, when filing for the new well owner in connection with a real estate transaction.
- Please note, although the GWS-11 PDF is no longer available for download, DWR will accept these forms completed prior to 8/1/2022, through 9/30/2022. Any GWS-11 Forms received after that date will be returned with instructions on how to resubmit them using the eForm.
