Four new eForms are now available on the eForms Dashboard. Private Driller and Private Pump Installer Assessments will be completed by well owners who intend to drill or repair their own wells or who intend on installing or replacing the pump in their own wells. Well Vault Installation will be completed to report the installation of wellheads in vaults in accordance with approved well construction variances. Rule 6.2.2.2 Notification will be used by licensed well contractors and well owners to report that work was completed on a well where they were unable to verify that the well had a valid permit. This form is not to be used for registering or permitting an existing well.
Any well constructed on or after January 1, 2026 to produce water for beneficial use must file a Well Construction and Yield Estimate Report that includes details on the well cleaning and development per Well Construction Rule 10.6. If you are submitting a paper or pdf GWS-31 for a well constructed on/after January 1, 2026 you will also need to submit a GWS-31A to report the well development and cleaning.
