We have released the Well Abandonment Report eForm to replace the GWS-09, GWS-09A and GWS-09B PDF Forms. You will need to file an individual Well Abandonment Report eForm for each well/hole. Please note, you will need to have an email address and create an account to file the eForm. Additional help can be found in the Guide to Filing the Well Abandonment Report eForm. Specific requirements on properly abandoning wells can be found in Rule 16 of the Well Construction Rules.
Please note that although the GWS-09 is no longer available for download, DWR will accept this form and bulk submittals through the previous process through October 31, 2024. Any GWS-09 PDF and/or paper forms received after that date will be returned with instructions on how to resubmit them using the eForm.
